Operating risk rarely stays still:
- exceptions replace the agreed workflow;
- local workarounds become the unofficial standard;
- a measure improves because its definition changed;
- key work concentrates around one experienced person;
- manager routines weaken as demand rises;
- workforce feedback accumulates without an owner or response;
- AI tools, information or audiences change while approval remains static;
- known issues stay open because authority or capacity is unclear.
A one-time project can establish a baseline and hand over improvements. It cannot freeze the organisation. Without continued attention, leadership may rediscover known weaknesses through disruption rather than evidence.
