A team can touch a high volume of work while accepted output moves slowly. People switch queues, wait for decisions, search for missing context, correct avoidable mistakes and ask the same expert for help. The effort is real, but much of it restores work that should have moved cleanly the first time.
The operating consequence may surface as missed service levels, ageing backlog, overtime, customer recovery, reduced capacity for priority work or managers pulled into constant escalation. A headcount request may follow even when the constraint is authority, access, instruction quality or process design.
Some checks and approvals protect the business. The question is which controls add value and which friction consumes time without an equivalent benefit.
