If the inquiry fits, the first conversation can clarify:

  • what broad evidence may already exist; and

Discovery is not a free audit, clinical consultation, legal opinion, funding assessment, technical certification or promise of results. You do not need to arrive with a root-cause analysis, complete dataset or selected programme.

A short business-level description is enough. For example: “A customer-support handoff across two time zones is creating repeat work and unclear ownership.”

Do not submit employee names, diagnoses, health or medical information, personal case details, allegations, passwords, security incidents, customer records, regulated data, confidential work product or files.

This form is not an emergency, grievance, whistleblowing, employment-case, data-breach or security-incident channel. Use the employer's approved HR, legal, privacy, safety, security or incident route as applicable. For immediate danger, contact local emergency services. Personal health support belongs with a qualified healthcare professional or an established employer support route.
